First Report of Injury or Illness Form
Complete the First Report of Injury or Illness form and fax to Versie Corum at (606) 598-1465 to file a claim.
KEMI Health Care Provider Card
A copy of the KEMI Health Care Provider Card will need to be provided to the provider in which the injured employee seeks treatment.
KEMI Prescription Card
A copy of the KEMI Prescription Card will need to be provided to the pharmacy in the event the injured employee is prescribed medications.
